Staff Training/Certificates-How to

In order to better serve our clients, provide the quality services, our staff request to finish all below trainings and send in certificate and documentations:

• Send in copy of a high school diploma or GED
• Send in copy of a valid Social Security Number
• Send in copy of a State of Ohio identification, a valid driver's license
• Submit BCI and FBI background check
  1. You can go to mason city policy department office to finish your BCI ($25).
     If you didn't continue to live in OH for past 5 years, you need FBI background check.
     Keep receipt. We will pay for this cost.

  2. You also can go your local city policy department office to finish your BCI and FBI background check and pay by yourself.

  1. Warren County DODD provider monthly free training
  
  You can attend warrant county free monthly training in person
  You must RSVP for this training to providerdevelopment@warrencountydd.org
  Seat is limited 10. Register as early as you can.

  2. Our agency provides First Aid, CPR class for employees. (Need to contact office to setup time based on instructor availability.)
  3. You also can go this Tailored Training Services LLC ($78) to finish the training
• Finish the online Direct Support Professional (DSP) initial training certificate
  1. OH DODD provide free online training for providers. Click here for how to set up the training.
• Car insurance card and Car insurance policy documentation with coverage page. (request one from your car insurance compnay)
• Resume and Reference list with at least 3 people
• Voided check or Bank letter/Direct depoist form for payroll company setting up directly deposit
