   Your log in ID is: your email

    Your password: ________

       you need to setup your profile after your load EVV app on your phone.
       Agency will add you to EVV system and email you that you are ready to setup.
       Check your email from Sandata, EVV will send you a temp password .

    Your EVV ID:_______you will get email before you start to work from our agency. (EVV ID will use for TVV log in call)


5. EVV log in/out error , email with below format. Office admin will use your email to correvt data and save your email for Medical.DODD audit:

   If you have issue with log in device or forgot to log in/out
   Please try to use TVV, call in to log your time.
   Or you can email office using below format:

    Example:
    Provider Employee Name/ID: 666
    Client Name: 123
    Date: 12-3, 2022
    Log in: 8:00 am
    Log out: 2:00 pm
    Reason: forgot to logout or other reasons
