Provider's Weekly and Monthly Documentation

OH DODD request HPC Provider to record they work data using weekly and monthly sheets.
Provider must use EVV to log in/out the work time for HPC hourly pay provider.
Provider paid by Medicaid every 15 minutes a unit.

Your timesheet must match EVV record.
15-minute increments per unit;
0 to 7 min = 0 Units
8 to 22 min = 1 Units
23 to 37 min = 2 Units
Each additional 15 minutes = 1 additional unit

      Provider needs to fill data sheets after log off to ensure the data accuracy.
      Provider needs to sign the sheets before turn in.
      Provider needs to turn in weekly sheet bi-weekly by every other Saturday.
      Provider needs to turn in monthly sheet at the end of the month.
      Provider needs to refer agency work calendar to tune in sheet on time.
